To register as a patient, initially you must enquire at reception. When you do so, you will be provided with a GPR form to fill out. When this is complete, you will be given an appointment for a New Patient Medical, to which you should bring the items which are specified on the “New Patient Registration” sheet provided to you.
If you fail to turn up for your appointment, are late or fail to bring your specified documents with you then we will be unable to proceed with your registration.
